The week after a missed Mounjaro dose: what to eat and what to expect
What to do about the missed injection itself, whether to take it late or wait for the next one, is a question for your prescriber or pharmacist, and we do not answer it. What we can help with is the food. As the level of Mounjaro in your blood falls, fullness fades and appetite comes back, often with the old food noise attached, and the week can turn into a large eating week that undoes a month of careful protein. The plan is to keep the four protein stops exactly where they were, let the extra appetite go into protein and vegetables rather than into whatever is nearest, keep the two strength sessions, and keep the plates small when the medication resumes, because side effects can behave as they did on a dose step.

What a gap in the medication does to your appetite
Mounjaro is tirzepatide, injected once a week. Blood levels peak within about one to three days of the injection and the half-life is about five days, so a missed dose does not switch the effect off overnight; it lets the level drift lower than usual over the following days. Fullness arrives later in a meal, the interest in food returns, and the stomach empties faster than it has for months. Most people notice it as a sudden ability to finish a plate, which feels like freedom on day one and like a problem by day four.
The risk to your muscle is not the extra food, it is what the extra food is made of and what stops during the week. Pooled across 20 randomised trials, 25.4% of the weight lost on tirzepatide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. A week of bread, biscuits and takeaway, with the strength sessions skipped because the routine slipped, moves the body composition in the wrong direction even if the scale barely moves. A week of larger protein meals, with both sessions done, is closer to a good week than a bad one.
Mounjaro's half-life is about five days, so appetite tends to return sooner after a missed dose than it does on semaglutide, often by day three or four. Have the protein meals ready before it does.

Keep the four protein stops, and make them bigger
Appetite is back, so the 30 g meals can become 40 g meals. Chicken, fish, eggs, yoghurt, lean mince. Protein first on the plate, then vegetables, then whatever else you want.
Eat from a plate, at a table, at the usual times
The week goes wrong through grazing between meals rather than through the meals themselves. Four sittings, protein first, nothing in between except water, tea and coffee.
Do both strength sessions, and enjoy them
This is the best-fed week you have had in months. Two 30-minute sessions with proper food around them will move the strength test more than any two sessions since you started.
Have the food ready by day three
Mounjaro's shorter half-life means appetite is often back by day three or four of a gap. Batch cook the chicken and the mince on day one.
A gap-week day at about 130 g of protein
- Breakfast: 3 eggs with 30 g (1 oz) cheese and one slice of toast, about 27 g
- Lunch: 150 g (5 oz) chicken breast in a large salad with a roll, about 46 g
- Afternoon: 200 g (7 oz) Greek yoghurt with fruit, about 20 g
- Dinner: 150 g (5 oz) salmon with potato and greens, about 37 g
Around 130 g on a day when eating is easy. Fill the rest of the plate with vegetables and a normal serve of carbohydrate, and drink water between meals. This is what a good eating week looks like without the appetite effect.

A gap week feels like the wheels coming off. These three numbers, taken on the usual day, show whether anything that matters has changed.
One strength test, same way every week
Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.
Average protein a day, whole week
Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.
Upper arm tape, same spot, relaxed
Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.

Will I gain weight in the week after a missed dose?
The scale often rises by a kilo or so (about 2 lb), mostly from more food and fluid in the system rather than fat. A week of protein-first meals and both sessions keeps the composition moving the right way, whatever the scale does for a few days.
