Coming off Ozempic: how to keep the weight off
The people who keep the weight off after Ozempic arrive at the last dose with their muscle intact, a protein habit that runs without hunger cues, and two strength sessions a week already in the calendar. Whether and how to stop is your prescriber's decision; what you do with your plate and your body either side of it is yours, and that is where the outcome is decided.
Your number, before anything else
Protein is what decides whether the weight you lose is fat or muscle. Work your number out once and we will keep it with you on every page here. No account, no email.
Saved in this browser only. Your dose and how you feel on Ozempic stay between you and your prescriber.
What happens when the medication stops
In the STEP 1 extension, people who stopped semaglutide regained about two thirds of what they had lost within a year. Appetite comes back, food noise comes back, and stomach emptying returns to normal within a few weeks of the last dose. None of that is failure.
What decides how much comes back is largely what your body is made of when you stop. Pooled across 20 randomised trials, 35.2% of the weight lost on semaglutide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. Muscle sets how much energy you burn at rest and how much you can eat without regaining; lose a large share of it and you stop with a smaller engine and a returning appetite.
Because Ozempic is often prescribed off-label for weight loss, supply and cost are common reasons people stop suddenly. If that might be you, start the step-down plan now rather than the week the script runs out.
What to measure instead of trusting the scale
Keep these going for a year after the last dose. They tell good regain from bad.
One strength test, same way every week
Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.
Average protein a day, whole week
Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.
Upper arm tape, same spot, relaxed
Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.
The step-down plan, starting three months out
Rebuild the muscle now, not after
If your strength test has dropped since you started, the last three months on the medication are the time to reverse it. Two 30-minute sessions a week, protein at 1.6 g per kg of goal weight (0.7 g per lb).
Set a weight range, not a weight
Pick a 3 kg (7 lb) band and weigh weekly. Touch the top of it and the response is two weeks of protein and training, not panic.
Keep the three numbers going for a year
Strength test, protein average, arm tape, every Sunday. Regain with strength climbing is a different problem from regain with strength falling.
Plan the first month off
Hunger returns before habits adjust. Meals at fixed times, protein first then vegetables then the rest, plate size fixed, no snacking after dinner. Expect a 1 to 2 kg (2 to 4 lb) water bounce in week one; it is not fat.
Do not let a script run out by accident
Ozempic is often stopped by supply or cost rather than choice. If that is likely, start the step-down plan now, and talk to your prescriber about a plan before the last pen, not after.
A maintenance day at 130 g for a 75 kg (165 lb) goal
- Breakfast: 2 eggs and 150 g (5 oz) Greek yoghurt: about 27 g
- Lunch: 150 g (5 oz) chicken or 200 g (7 oz) tofu on a big salad with olive oil: about 42 g
- Afternoon: 30 g (1 oz) nuts and 100 g (3.5 oz) cottage cheese: about 18 g
- Dinner: 150 g (5 oz) fish or lean meat, 150 g (5 oz) vegetables, 100 g (3.5 oz) potato or rice: about 38 g
About 130 g and a normal amount of food. Maintenance is a fuller plate with the same protein rule, not a smaller deficit.
Work out your own target on OzempicThe decisions that are your prescriber's
- Whether to stop, come down gradually, or stay on the medication. That is theirs, and it should never happen because of supply or cost without a conversation
- Rapid regain in the first three months off, especially with blood sugar or blood pressure rising
- Hunger that feels unmanageable
Dose and timing on Ozempic are your prescriber's decisions; we coach the food and the strength.
Questions people ask
Will we regain the weight after stopping Ozempic?
In the trials, most people regained a large share within a year of stopping. The people we coach who hold their weight kept their muscle and protein habit, and started training before the last dose.
How long before the last Ozempic dose should we start preparing?
Three months: long enough to rebuild a dropped strength test, fix the protein habit and practise maintenance eating while the medication is still helping.
What happens to our appetite when we stop Ozempic?
It returns over a few weeks as the medication clears, usually before your habits have adjusted. Fixed meal times and protein first are what carry you through that gap.
The evidence, and how current it is
What comes back is not what came off. That is the single most useful thing to know before your last dose.
Weight regained after stopping appears to come back disproportionately as fat, so repeated cycles can leave someone heavier in fat and lighter in muscle than when they started. Bosomworth NJ, Canadian Family Physician 2025
Adding resistance training brought the lean share of weight lost down to 17.5%, the most favourable result of any approach studied. Eisa N et al, Diabetes, Obesity and Metabolism 2026
Guideline-informed protein targets during this kind of weight loss start at 1.2 g per kg of body weight a day (0.55 g per lb). Arslan S, Clinical Nutrition ESPEN 2026
