Menopause and Ozempic: protecting muscle and bone
Menopause and a GLP-1 pull in the same direction on muscle. Oestrogen falling is already associated with losing lean mass and bone, and a large calorie deficit on Ozempic adds to both. The response is not to lose more slowly and hope. It is protein at the top of the range, 1.6 g per kg of goal weight, two 30-minute strength sessions a week that load the hips and spine, and enough calcium and vitamin D. Hormone therapy, bone density scans and blood tests are conversations for your doctor.

The weekly plan through menopause
Most women we coach on Ozempic are in their fifties, and the pattern is consistent: weight falls steadily, strength falls quietly, and nobody measures the second one. The weekly sit-to-stand is the cheapest protection there is.
Protein at 1.6 g per kg of goal weight, not the floor
For a 65 kg (143 lb) goal, that is about 104 g a day, in 30 to 40 g servings. Spreading it across three or four meals matters more after 45 than it did before, because a single large serve does not do the work of two medium ones.
Two strength sessions that load hips and spine
Squat pattern, hinge, push, pull, carry, core brace. Loaded carries and hinges are the ones that matter for hips and spine. Machines, dumbbells or bands all work; the load has to be enough that the last two reps are hard.
Cover calcium and vitamin D from food, and ask about testing
Dairy, tinned fish with bones, tofu set with calcium, fortified milks. Around three serves a day for most adults. Ask your doctor about vitamin D testing and whether a bone density scan is appropriate for you.
Treat sleep and hot flushes as part of the plan
Broken sleep in a deficit shifts more of the loss towards lean tissue and wrecks the quality of your sessions. Cool room, earlier dinner, less alcohol, and raise persistent symptoms with your doctor rather than enduring them.
Set the weekly strength test on a fixed day
Sunday morning, 30-second sit-to-stand, same chair. On Ozempic the loss is gradual enough that only a trend reveals a problem, and three weeks of falling numbers is your early warning.
Two things happening to muscle at once
From the mid-forties onward, muscle is already harder to keep. Oestrogen decline is associated with loss of lean mass and of bone density, and the body responds less strongly to a given amount of protein than it did at thirty. That is why the protein target we use is set against goal weight and sits at 1.2 to 1.6 g per kg (0.55 to 0.7 g per lb), with women over 45 usually better served near the top of it.
Now add the medication. Pooled across 20 randomised trials, 35.2% of the weight lost on semaglutide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. A deficit large enough to drive fast weight loss is also a signal to shed tissue that is expensive to maintain, and muscle is exactly that. On semaglutide the appetite drop tends to work like a dimmer switch rather than an on-off: meals feel possible but small, and fullness arrives early and stays. Most people we coach on Ozempic can eat a modest plate, then nothing for hours. The women we coach who come through this well are not the ones who lost the most weight. They are the ones whose strength test went up while the scale went down.
A day at 1.6 g per kg for a 65 kg goal
- Breakfast: 200 g (7 oz) Greek yoghurt with berries and seeds, about 22 g protein and a calcium serve
- Lunch: 120 g (4 oz) tinned salmon with bones on wholegrain toast, about 32 g, calcium and vitamin D together
- Afternoon: 30 g scoop whey in 250 ml (8.5 fl oz) milk, about 33 g and another calcium serve
- Dinner: 120 g (4 oz) chicken thigh with lentils and greens, about 38 g

Through menopause the scale is especially misleading. These three tell you what you are keeping.
One strength test, same way every week
Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.
Average protein a day, whole week
Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.
Upper arm tape, same spot, relaxed
Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.
Worth a conversation with your doctor
- Hormone therapy, whether it suits you and how it fits with weight loss, which is a medical decision and not a coaching one
- Bone density screening, vitamin D and iron testing, particularly with a family history of osteoporosis or a previous fracture
- Any unexpected bleeding, severe hot flushes, low mood or changes to your cycle, which deserve proper assessment

Questions people ask
Does menopause change what we should do on Ozempic?
The medication itself is your prescriber's area. What differs is the body it is acting on: lean mass and bone are already under pressure from falling oestrogen, so the cost of losing weight without protein and strength work is higher than it would have been at thirty.
How much protein do I need on Ozempic after menopause?
We aim for the top of the range, around 1.6 g per kg of goal weight (0.7 g per lb), spread across three or four servings of 30 to 40 g. If you have kidney disease, your doctor sets your target instead.
What should we watch on Ozempic through menopause?
Prescribing decisions belong with your doctor. What we can say is that whatever the cause of the weight, the muscle and bone risk during menopause makes protein and strength training more important, not less.
Where these numbers come from
Two forces pull on the same tissue here. These are the figures for what pushes back, including the one that says protein alone will not.
