Reflux and burping on Ozempic: what to eat instead

Reflux, burping and the burps that smell of egg all come from the same thing on Ozempic: food sitting in the stomach far longer than it used to. Fat sits longest, volume makes it worse, and lying down within three hours of a meal turns it into heartburn. Smaller, lower-fat, protein-first meals eaten earlier settle it for most people we coach. On Ozempic reflux is worst in the first two or three days after the injection, when blood levels peak. Keeping those days lower in fat and smaller in volume usually removes most of it without changing anything else.

A low-reflux, protein-first day

  • Breakfast: two poached eggs with one slice of toast, about 15 g protein, no butter
  • Mid-morning: 170 g (6 oz) low-fat Greek yoghurt, about 17 g
  • Lunch: 100 g (3.5 oz) grilled chicken with rice and steamed vegetables, about 31 g
  • Dinner by 6pm: 100 g (3.5 oz) white fish with potato and greens, about 23 g, nothing fried

Around 86 g across four small, low-fat plates, with the last one three hours before bed. If you need more, add a shake made with lactose-free or low-fat milk between meals.

A slow stomach and a full one

Ozempic slows stomach emptying. That is part of how it works, and it is also why a normal meal now behaves like a very large one. Fat is the slowest thing to leave the stomach, so a creamy or fried meal can still be there hours later. Gas builds behind it, which is the burping, and the pressure pushes acid up, which is the heartburn. On semaglutide the appetite drop tends to work like a dimmer switch rather than an on-off: meals feel possible but small, and fullness arrives early and stays. Most people we coach on Ozempic can eat a modest plate, then nothing for hours.

The burps that smell of sulphur are the same story taken further: food fermenting in a stomach that has not emptied. They are unpleasant rather than dangerous, and they almost always follow a large or fatty meal. The reason we care is what people do next. The usual response is to stop eating, and skipping meals is how protein falls under the floor. Pooled across 20 randomised trials, 35.2% of the weight lost on semaglutide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added.

Changes that settle it within a week

  1. Cut the fat in the meals that cause it

    Swap fried for grilled, cream sauces for tomato, and full-fat cuts for lean. Protein itself is not the problem; the oil it is cooked in usually is. Lean chicken, white fish, egg whites, low-fat dairy and lentils all move through faster.

  2. Halve the volume, double the meals

    Four small plates beat two normal ones on Ozempic. Aim for 20 to 25 g of protein in each rather than 40 g in one hit, and stop at comfortably fed rather than full.

  3. Three hours upright after eating

    No lying down, no reclining chair, no bed. Raise the head of your bed by 10 to 15 cm (4 to 6 in) with blocks or a wedge, which works better than stacking pillows.

  4. Drop the fizzy drinks and drink between meals

    Carbonated drinks add gas to a stomach that is already slow. Take your fluid between meals rather than with them, so the meal itself takes up less room.

  5. Make the two days after your shot the lean days

    Ozempic peaks one to three days after the injection. Plan grilled chicken, white fish, egg whites and low-fat yoghurt for those two days, and keep anything rich for later in the week.

How you will know it is working

Reflux makes people eat less, and eating less is what costs muscle. These three catch it early.

  1. One strength test, same way every week

    Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.

  2. Average protein a day, whole week

    Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.

  3. Upper arm tape, same spot, relaxed

    Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.

  • Chest pain, pain spreading to the arm or jaw, or breathlessness, which needs urgent medical attention rather than a diet change
  • Reflux most nights, trouble or pain swallowing, food sticking, vomiting blood or black stools, or unexplained symptoms lasting more than two weeks

Does Ozempic cause acid reflux?

Reflux and burping are commonly reported, and the mechanism is clear: food sits in the stomach longer. Meal size, fat content and timing are the three things that change it. Persistent reflux still needs your prescriber.
