Restarting Saxenda after a break: getting back on track
Restarting is common, and whatever happened during the break, the weight coming back was the expected outcome rather than a personal failure. What matters now is that the restart is not a repeat of the first round. Last time the muscle usually went with the fat, and without protein and strength work the same thing happens again, on a body that already has less to give. Your prescriber decides the medication and how it restarts. Your job is four protein stops a day and two sessions a week, from week one rather than from month three.

Why the weight came back, and what that means for this attempt
In the SCALE trials, weight began returning in the weeks after liraglutide was stopped, consistent with what has been seen across the class. The pattern in the trial extensions is consistent, and it is a statement about the medication rather than about willpower. What is less often said is what comes back: regain is mostly fat, while the lean mass lost during the first round does not return on its own. Pooled across 20 randomised trials, 26.8% of the weight lost on liraglutide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. That is why a second round on the same plan tends to leave you with a slightly worse body composition than the first.
Liraglutide's appetite effect is milder and steadier than the weekly medications. Most people we coach on Saxenda can eat a normal small meal; the risk is subtler, because the appetite that remains is often for toast rather than for chicken. The practical effect is that appetite suppression returns quickly, usually inside a fortnight, and the eating gets small again just as fast. Whether the protein comes with it is the entire difference between this round and the last one.
Restarting a daily medication after a break has its own schedule and it is your prescriber's to set. The food side does not change: four stops, 25 to 35 g each.
The first fortnight back

Restarting after a break is a clinical decision, including where in the schedule you restart. It is not a conversation to have with a forum or with the pen you did not finish.
Set the four protein stops on day one
Do not wait for appetite to shrink before building the structure. Four stops of 25 to 35 g at fixed times, from the first week, is the single change that makes this round different.
Book the two sessions before the appetite drops
Week one is the easiest week to start training, because you still have energy and food. People who wait until they feel ready mostly never start.
Take the three numbers in week one so you have a baseline
Strength test, protein average, waist. In three months you will want to know whether this round protected what the last one did not.
Rebuild the daily routine first
Saxenda is taken every day, so the eating rhythm can hang off a fixed daily pattern. Decide the four times in week one and keep them.
A restart day at about 110 g of protein
- Breakfast: two eggs with wholegrain toast and a glass of milk, about 28 g
- Mid-morning: 170 g (6 oz) Greek yoghurt, about 17 g
- Lunch: 120 g (4 oz) chicken or tofu with rice and vegetables, about 32 g
- Dinner: 120 g (4 oz) salmon, beef or lentils with potato and salad, about 32 g
Around 110 g while appetite is still workable. Keep the same structure as the portions shrink, and switch the mid-morning stop to a shake on the day yoghurt stops going down.

You have been here once already. These three are how you tell whether the second round is going better than the first.
One strength test, same way every week
Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.
Average protein a day, whole week
Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.
Upper arm tape, same spot, relaxed
Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.
Questions people ask
What is different about starting Saxenda again?
The medication side of it belongs with your prescriber. What is different on our side is the starting point, because the lean mass lost in the first round does not come back on its own, so protein and strength work matter more this time than they did last time.
Do I start where I left off?
That is your prescriber's decision and it depends on how long the break was. Restarting on your own at the dose you finished on is exactly the thing that produces a fortnight of nausea.
Do I need to step Saxenda back up after a break?
Quite possibly, and that is your prescriber's call based on how long you were off. Restarting at your old daily dose without asking is how people end up with a fortnight of nausea.
