On Saxenda over 60: keeping the muscle you have left
Over 60 the maths changes. You are already losing muscle at perhaps 1% a year, your body responds less strongly to a given meal of protein, and a GLP-1 deficit can take lean mass quickly if nothing pushes back. The plan is more protein per meal, not more meals: 30 to 40 g at each of three or four sittings, totalling 1.6 g per kg of goal weight, plus two 30-minute strength sessions a week. Any fall, any new weakness, or weight coming off faster than planned goes to your doctor.

Saxenda's slower loss suits some older patients better, which is a discussion for your doctor. The protein and training plan is unchanged: 30 to 40 g per meal, two sessions a week.
A day built on 30 g blocks
- Breakfast: 3 scrambled eggs with cheese on toast, about 30 g protein, soft and easy to chew
- Lunch: 120 g (4 oz) tinned salmon or tuna with potato salad, about 32 g plus calcium if the bones are in
- Afternoon: 30 g scoop whey in 250 ml (8.5 fl oz) milk, about 33 g
- Dinner: 120 g (4 oz) slow-cooked beef or lentil casserole with vegetables, about 35 g
Around 130 g in four blocks, each large enough to trigger the building response. Nothing here needs a strong jaw or an hour at the stove.

Two things are working against you before the medication is involved. Muscle mass declines gradually from midlife, and the muscle you have responds less strongly to a meal than it did at thirty, so a 15 g serve of protein that once did the job now does very little. That is why we set per-meal targets rather than daily ones: 30 to 40 g in a sitting is the amount that reliably triggers the building response in older adults.
Then the deficit arrives. Pooled across 20 randomised trials, 26.8% of the weight lost on liraglutide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. Liraglutide's appetite effect is milder and steadier than the weekly medications. Most people we coach on Saxenda can eat a normal small meal; the risk is subtler, because the appetite that remains is often for toast rather than for chicken. In people over 60 the cost of that is not cosmetic. It is the ability to get out of a chair without using your hands, to carry shopping, to catch yourself when you trip. The weight loss is worth having. Losing it along with the strength that keeps you independent is not a trade worth making, and it is avoidable.
The weekly plan after 60
30 to 40 g of protein per meal, three or four times
Not 100 g spread thinly. For a 70 kg (154 lb) goal weight, aim at about 112 g a day in servings of 30 to 40 g. Eggs, fish, chicken, mince, dairy, tofu and legumes, with a shake to fill the gap.
Two 30-minute strength sessions, sitting or standing
Squat pattern, hinge, push, pull, carry, core brace. A chair sit-to-stand, a band row, a wall or bench push-up and a suitcase carry cover all of it. Hold a chair back for balance if you need to.
Make chewing easier, not protein optional
Mince, fish, eggs, slow-cooked meat, dairy, tofu and soups with lentils go down when steak will not. If dentures or dental pain are the barrier, see a dentist rather than quietly dropping protein.
Drink to a schedule and watch for dizziness on standing
Thirst is less reliable after 60, and intake is already low. Around 2 litres (68 fl oz) a day. Light-headedness on standing is common with fast weight loss and worth telling your doctor about, especially if you take blood pressure medication.
Use the steadier appetite for real meals
Saxenda's milder effect means most people over 60 can still eat normal food. Take the chance to build three 30 g meals into the day while it is easy.
How you will know it is working
After 60 the strength test is not a fitness metric, it is an independence metric.
One strength test, same way every week
Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.
Average protein a day, whole week
Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.
Upper arm tape, same spot, relaxed
Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.
Worth a conversation with your doctor
- Any fall, near fall, or new unsteadiness on your feet, which deserves a proper review rather than caution alone
- Weight coming off faster than you and your doctor planned, or a loss of appetite so complete that you are eating almost nothing
- A review of all your medications together, since blood pressure, diabetes and heart medications often need adjusting as weight comes down

Is Saxenda safe over 60?
That is your doctor's judgement, based on your other conditions and medications. What we can tell you is that over 60 the muscle side of weight loss needs active protection, and that is the part we coach.
How much protein should I eat over 60 on Saxenda?
Around 1.6 g per kg of goal weight (0.7 g per lb), in servings of 30 to 40 g. Older muscle responds less to small amounts, so a big breakfast beats four snacks. If you have kidney disease, your doctor sets the number.
Is daily Saxenda easier to manage after 60?
Some people prefer the steadier effect, and some find a daily injection harder to keep up with. Your doctor decides what suits you. The protein and training plan is the same either way.
What the evidence actually says
After 60 the per-meal figure matters more than the daily one, and the first line is the limitation we would rather you heard from us.
In 191 older adults, higher protein was associated with more preserved lean tissue but not with better grip strength, walking speed or chair-stand scores. Protein looks after the material. Training is what turns it into function. Eglseer D et al, Nutrition Journal 2026
