Feeling weak on Wegovy: how to tell if you are losing strength, and what to do

Weakness on Wegovy has two common causes: eating too little (fatigue, dizziness, low fuel) and losing muscle (a strength test that drops week after week). Do the 30-second sit-to-stand test today and again in a week; if the number is falling, muscle is going, and protein at 1.2 to 1.6 g per kg of goal weight (0.55 to 0.7 g per lb) plus two short strength sessions a week is the fix.

Weak versus weaker: the difference matters

Feeling weak is a sensation. Getting weaker is a measurement. On Wegovy the first is nearly universal in the early months, because you are eating half of what you used to and your blood sugar is steadier and lower. That kind of weak is tiredness, and it improves with regular protein meals and fluids.

Getting weaker is different. Pooled across 20 randomised trials, 35.2% of the weight lost on semaglutide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. Muscle is what you stand up with, carry shopping with, and get off the floor with. In the people we coach, the ones who notice stairs getting harder at month three or four are almost always under the protein floor and not doing any resistance work. Fat loss does not make you weaker. Muscle loss does.

At the top Wegovy step, intake is often under 1,000 calories (4,200 kJ) a day, and nobody can train well on that. The fix is not to train harder; it is to get protein and total food up so that the two short sessions are possible.

This week's test and fix

  1. Do the sit-to-stand test today

    A standard chair, arms crossed, count how many full stands in 30 seconds. Write it down. Repeat every Sunday. This number is your muscle gauge.

  2. Two 30-minute sessions, this week

    Six movements: sit-to-stand or goblet squat, hip hinge, a push (wall or incline push-up), a pull (band row), a carry (two shopping bags), and a core brace. Ten to twelve reps, two sets, done. Home or gym.

  3. Protein floor, then more

    1.2 g per kg of goal weight minimum, 1.6 if you are training. For a 65 kg (143 lb) goal that is 78 to 104 g. Thirty grams in the two hours after training.

  4. Eat something before you train

    Half a shake or a yoghurt 30 to 60 minutes before. Training on nothing on Wegovy is what makes people feel faint and then stop training.

  5. Get food up before intensity

    At the top step, you cannot train well on 900 calories (3,750 kJ). Add a 30 g protein meal before you add a heavier session; the session will go better and you will keep doing it.

A training-day plate at 120 g

  • Pre-session: 150 g (5 oz) Greek yoghurt: about 15 g
  • Post-session: shake with 250 ml (8.5 fl oz) milk: about 30 g
  • Lunch: 120 g (4 oz) chicken or 150 g (5 oz) tofu with vegetables: about 35 g
  • Dinner: 150 g (5 oz) fish or lean mince: about 35 g
  • Evening: 100 g (3.5 oz) cottage cheese if there is room: about 12 g

Around 127 g. On non-training days drop the pre-session snack and keep everything else.

When weakness needs your prescriber

  • Dizziness or fainting when you stand, or a racing heart
  • Weakness on one side, in one limb, or with numbness (seek urgent care)
  • Persistent fatigue with cold intolerance or breathlessness (iron, B12 and thyroid are worth a test)
  • Whether your dose is contributing to how little you are eating. That is their decision

The three numbers that tell you what you are keeping

Weak is a feeling. Weaker is a number. These three turn one into the other.

  1. One strength test, same way every week

    Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.

  2. Average protein a day, whole week

    Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.

  3. Upper arm tape, same spot, relaxed

    Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.

Questions people ask

Why do we feel weak on Wegovy?

Most often because you are eating much less, with lower and steadier blood sugar. That is tiredness and it improves with regular protein meals. If your strength test is dropping too, you are losing muscle, which needs protein and training.

Can we build muscle on Wegovy?

You can certainly keep it, and many people we coach who have never trained before get stronger while losing weight, because they are starting from zero. Two short sessions a week is enough.

Can we exercise at the top Wegovy step?

Yes, and you should, but lightly if intake is very low. Two 30-minute sessions with a pre-session snack. Fainting or dizziness during exercise goes to your prescriber.
