Injection day on Zepbound: what to eat before, during and after

Eat your biggest protein meal of the week before your Zepbound injection, when the level in your system is lowest, then go small, cold and protein-first for the 24 to 48 hours after it. Blood levels peak within about one to three days of the injection, so that is your window for shakes, yoghurt, eggs and tinned fish rather than a roast dinner.

Your number, before anything else

Protein is what decides whether the weight you lose is fat or muscle. Work your number out once and we will keep it with you on every page here. No account, no email.

The two-day injection plan

Zepbound's cycle is the same as Mounjaro's: the day before injection is your big-meal day and the two after are your cold-protein days. Once the pattern is known, most people we coach plan their social meals around it.

  1. The day before: eat well

Your biggest protein meal of the week. 180 to 200 g (6 to 7 oz) of meat or fish, or the equivalent, plus vegetables. Bank it.

  2. Injection day: normal morning, light evening

Inject at a consistent time. If you inject in the evening, eat a proper protein meal beforehand. If in the morning, keep the day's meals small and cold.

  3. Day one after: five small cold stops

Yoghurt, a shake, boiled eggs, tinned tuna, cottage cheese. About 20 g each, three hours apart. No frying, no strong smells. On day two, rebuild to three meals starting with something warm but plain, like chicken soup.

  4. Train the day before, or two days after

Not the day after. That is the lowest-energy point of the week for most people, and a bad session then is the one that makes people quit.

  5. Plan social meals around the cycle

Once you know your Zepbound pattern, put dinners out and family meals on the day before injection and never on days one and two after. It removes a lot of stress.

What the injection cycle does to your appetite

Zepbound is tirzepatide, injected once a week. Blood levels peak within about one to three days of the injection, then levels fall gradually until the next dose, with a half-life of about five days. That gives most people a rhythm: appetite and side effects at their strongest one to three days after injecting, and at their mildest in the day before the next one. In SURMOUNT-1, nausea was reported by roughly a quarter to a third of people at the higher tirzepatide doses and constipation by about one in six, mostly during dose escalation.

The rhythm is useful once you know it. The day before injection is your best eating day; use it for the roast, the big steak, the meal out. The two days after are for the low-effort protein you can get down without a smell or a fuss. Pooled across 20 randomised trials, 25.4% of the weight lost on tirzepatide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. Those two hard days a week, badly handled, are where a lot of it goes.

Day after injection: 90 g in five cold stops

  • 08:00: 170 g (6 oz) Greek yoghurt: about 17 g
  • 11:00: shake, 250 ml (8.5 fl oz) milk plus protein powder: about 30 g
  • 14:00: 2 boiled eggs: about 13 g
  • 17:00: 95 g (3.5 oz) tin of tuna: about 20 g
  • 20:00: 100 g (3.5 oz) cottage cheese: about 12 g

About 92 g. Nothing needs cooking. Nothing has a smell. That is the point.

Three numbers, every Sunday

Take the strength test and the tape on your best day of the cycle, the same day every week, so the cycle does not skew them.

  1. One strength test, same way every week

Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.

  2. Average protein a day, whole week

Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.

  3. Upper arm tape, same spot, relaxed

Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.

Prescriber questions, not coaching questions

  • Vomiting on injection day that lasts beyond 24 hours, or you cannot keep fluids down
  • A missed dose, or a question about changing the day or time you inject
  • Injection-site reactions that are red, hot or spreading

Dose and timing on Zepbound are your prescriber's decisions; we coach the food and the strength.

Forty foods, ranked by protein per bite

The hard part is not knowing you need protein on Zepbound, it is hitting the number on a day when food is the last thing you want. One page, grams and ounces, what a real serve looks like, and the first-bite rule for when appetite is gone. Print it and put it on the fridge.

Then five short emails over five days on keeping muscle while the weight comes off, and a short note every fortnight after that if you want it. One click to stop, any time.

Questions people ask

Should we eat before our Zepbound injection?

Yes. The level in your system is lowest right before the next dose, so that is your best appetite of the week. A full protein meal beforehand sets you up for the two harder days.

What should we eat the day after our Zepbound injection?

Small, cold, plain, protein-first: yoghurt, a shake, boiled eggs, tinned fish, cottage cheese, every three hours. Save hot, fatty and strong-smelling meals for later in the week.

Can we exercise on Zepbound injection day?

Yes, ideally before the injection or the day before. The day after is the lowest-energy point for most people; keep it to a walk.

Where these numbers come from

Small meals on injection day still have to clear the thresholds below, which is why what you eat matters more than how much.

Spread across meals at roughly 0.3 to 0.4 g per kg per meal, rather than taken mostly at dinner. Arslan S, Clinical Nutrition ESPEN 2026

Each of those meals wants roughly 2.5 to 3 g of leucine, the amino acid that actually switches muscle building on. That is about 30 g of whey, 120 g of chicken breast, or four eggs. Arslan S, Clinical Nutrition ESPEN 2026
