Going back to exercise after a long break on Zepbound
The mistake nearly everyone makes in the first week back is doing what they used to do. Start well below what you think you can manage: two sessions a week, six movements, one or two sets each, at a weight you could lift twice as many times. Why the first fortnight goes wrong
Strength comes back faster than tendons and connective tissue adapt. The muscle remembers, the joints do not, and that gap is why an enthusiastic first week so often ends with something sore for a fortnight. Add a large calorie deficit and recovery slows again. Tirzepatide is the medication where people most often describe a very short appetite window: a few bites, then done. Eating very little and training the way you did at thirty is the combination that produces injuries.
The other thing that goes wrong is the measure. People restart aiming at how they look or what the scale says, both of which move slowly and neither of which reflects what the session did. Pooled across 20 randomised trials, 25.4% of the weight lost on tirzepatide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. What you are buying with two sessions a week is the lean mass the medication does not protect on its own, and the way to see it is a repeatable strength test rather than a mirror.

What to measure instead of trusting the scale
Starting again is easy to abandon, because nothing visible happens quickly. These three show progress inside a fortnight.
One strength test, same way every week
Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.
Average protein a day, whole week
Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.
Upper arm tape, same spot, relaxed
Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.
Weeks one to eight, written down
Weeks one and two: learn the six shapes with almost no weight
Squat pattern, hinge, push, pull, carry, brace. One set of each, ten repetitions, a weight that feels easy. The goal is to finish the week with nothing sore.
Weeks three and four: add a second set
The same six movements, two sets, slightly more weight on whichever felt easiest. This is where most people want to jump ahead, and where holding back pays.
Weeks five to eight: add load, not exercises
Three sets on the big three movements, one set on the rest, and a small amount of weight added each week. Six movements is enough for a very long time.
Take one strength test on day one and repeat it monthly
Sit-to-stands in thirty seconds, push-ups, or goblet squat repetitions at a fixed weight. That number rising is the proof you are keeping muscle rather than only losing weight.
Eat something ninety minutes before
On Zepbound you will not feel like it, and the session will be poor without it. A yoghurt and a banana is enough to change the whole thirty minutes.
The whole first session, about twenty five minutes
- Sit-to-stand or goblet squat, 1 set of 10
- Hip hinge with a broom handle, then light dumbbells, 1 set of 10
- Push-up against a bench or wall, 1 set of 8, and a row with a band or dumbbells, 1 set of 10
- A carry with a shopping bag in each hand for 30 metres (about 33 yards), twice, then a plank or dead bug for 20 seconds, twice
If you finish and feel you could do the whole thing again, that is the correct first session. Add the second set next fortnight. The people still training in month six are the ones whose first week felt too easy.

How do I start exercising again on Zepbound after years off?
Two thirty-minute sessions a week, six movements, one set each for a fortnight, at a weight that feels easy. Add sets before weight, and weight before you add exercises.
Should I do cardio or weights?
Weights, if there is only time for one. Cardio is good for your heart and it does not hold muscle in a deficit. Walk as well if you can, but the two strength sessions are the part that protects what you are trying to keep.
Do I need a gym to restart on Zepbound?
No. A chair, a pair of dumbbells or two shopping bags, and a resistance band cover all six movements. What matters is that the load rises over the weeks, not where you do it.
