Constipation that will not shift on Ozempic: what to change next

If you have added fibre and water and nothing has moved for days, the usual advice has already failed you and repeating it will not help. The three changes that most often break a stuck pattern on Ozempic are eating more total food rather than more fibre, moving every day, and asking a pharmacist which product suits you alongside your other medications. Why the standard advice stops working

Fibre needs volume and fluid to do its job. On Ozempic you are eating a fraction of what you used to, so a fibre supplement in a nearly empty gut can bulk without moving, and people often feel worse rather than better. On semaglutide the appetite drop tends to work like a dimmer switch rather than an on-off: meals feel possible but small, and fullness arrives early and stays. Most people we coach on Ozempic can eat a modest plate, then nothing for hours. The pattern we see most is someone eating six or seven hundred calories a day, drinking plenty, taking a fibre powder, and wondering why nothing has happened in five days.

In the STEP 1 trial of semaglutide, roughly four in ten people reported nausea at some point and about one in four reported constipation, most of it clustering in the weeks after a dose step. The other half of it is transit. The medication slows the whole tract, not only the stomach, which is why a daily walk, a warm drink first thing and enough total food tend to do more here than another scoop of psyllium.

On Ozempic the pattern often tracks the week: slowest in the two or three days after the shot, easier by day five or six. If yours has stopped tracking the week at all, mention that at your next appointment.

What to measure instead of trusting the scale

Constipation resolves faster than muscle does. These three keep the slower problem in view while you fix the urgent one.

  1. One strength test, same way every week

    Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.

  2. Average protein a day, whole week

    Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.

  3. Upper arm tape, same spot, relaxed

    Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.

What to change, in order

  1. Eat more total food before you add more fibre

    A gut with almost nothing in it has nothing to move. A fourth small stop, even 150 calories of yoghurt or a shake, does more for most people than doubling a supplement, and it protects muscle at the same time.

  2. Get fluid in with something in it

    Plain water is easy to forget when you are not hungry. Milk, soup, broth and shakes all count as fluid and bring protein with them. A glass at every meal and one in between is the target.

  3. Move every day, even briefly

    Ten minutes of walking after your two biggest meals is the cheapest item on this list and the one most people skip. Movement drives transit. Sitting still does not.

  4. Ask a pharmacist which product suits you

    This is the point where a product is reasonable, and a pharmacist can tell you which one fits alongside your other medications. We do not name doses on this site, and neither should anyone else who has not met you.

  5. Front-load food onto your easier days

    Ozempic peaks one to three days after the injection. Use days four to seven to eat and drink a bit more, and the slow days have something to work with.

A day built for transit that still carries protein

  • Breakfast: 200 g (7 oz) Greek yoghurt with two tablespoons of oats and berries, about 22 g
  • Mid-morning: a warm drink and a ten-minute walk, which does more for transit than another supplement
  • Lunch: lentil and vegetable soup with a boiled egg, about 20 g, plus a full glass of water
  • Dinner: 120 g (4 oz) chicken or fish with potato, a teaspoon of olive oil and two vegetables, about 32 g

Around 75 to 90 g of protein, four points of fluid, some fat and two short walks. If your target is higher than that, close the gap with a shake rather than a bigger plate.

When this stops being a food question

  • Several days without a bowel movement together with abdominal pain, a firm swollen belly, or vomiting, which needs urgent medical attention
  • Blood in the stool, or a change in pattern that persists after the first few weeks on a new dose

Questions people ask

Why has fibre made my constipation worse on Ozempic?

Fibre needs food volume and fluid to work. If you are eating very little, a bulking fibre can sit rather than move. Eating a bit more total food, and drinking with it, usually helps more than another scoop.

How long should constipation last on a GLP-1?

It often settles within a couple of weeks of a dose step. What is not usual is several days with pain, a hard swollen belly or vomiting, and that is a same-day call rather than a food adjustment.
