How much water to drink on Ozempic: the fluid and salt rule
Aim for around 2 litres (68 fl oz) of fluid a day on Ozempic, more in heat or after training, and take it on a schedule rather than waiting for thirst, because thirst is an unreliable prompt when you are eating very little. Half the reason is that food used to supply a surprising share of your daily fluid, and it no longer does. The other half is salt: low intake means low sodium, and low sodium with plenty of plain water is the combination behind most of the headaches, cramps and light-headedness we hear about. Water, tea, coffee, milk and broth all count. Dark urine all day, dizziness that does not settle, or vomiting or diarrhoea that stops you keeping fluid down belong with your prescriber.

Why you are short of fluid without noticing
On semaglutide the appetite drop tends to work like a dimmer switch rather than an on-off: meals feel possible but small, and fullness arrives early and stays. Most people we coach on Ozempic can eat a modest plate, then nothing for hours. Meals are where a large share of daily fluid used to come from: soup, fruit, vegetables, milk on cereal, the glass of water that came with dinner. Eat a third of the food and you have quietly lost a third of that fluid, and the medication removes the thirst cue along with the hunger one. Add the fluid that leaves with nausea, loose stools or a hot day, and the margin is gone by mid-afternoon.
Sodium goes the same way. Salt arrives with food, and a day of a shake, a yoghurt and half a chicken breast carries very little of it. Low fluid and low sodium together produce the headache behind the eyes, the afternoon flatness, the night cramps and the wobble on standing that people blame on the medication. Pooled across 20 randomised trials, 35.2% of the weight lost on semaglutide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. Hydration does not change those figures directly, but a dehydrated body trains badly, eats badly and recovers slowly, and those three do.
On Ozempic the fluid gap is widest in the two or three days after the injection, when both food and thirst are at their lowest. Those are the days to set the bottle out and add the salted broth.
What to measure instead of trusting the scale
Fluid swings the scale by a kilo (2 lb) from one morning to the next and says nothing about muscle. These three numbers read through it.
One strength test, same way every week
Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.
Average protein a day, whole week
Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.

Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.
The fluid and salt rule, in practice
Set the 2 litres (68 fl oz) out where you can see them
A 1 litre (34 fl oz) bottle filled twice, or a 750 ml (25 fl oz) bottle filled three times, finished by the evening. Counting glasses does not work when nothing inside you is asking for them.
Count everything that is wet
Water, tea, coffee, milk, broth, a shake, sparkling water without sugar. Milk and a shake count twice, because they bring protein with the fluid. Alcohol does not count, and costs you fluid.
Salt your food normally
Unless your prescriber has told you to limit sodium for blood pressure, heart or kidney reasons, in which case follow them, not us. A cup of salted broth on a low-food day, or a rehydration sachet after loose stools, covers what the food no longer does.
Drink between meals, not on top of them
A big glass with a meal adds volume to a stomach that is emptying slowly and brings the fullness on sooner. Sip through the day, stop twenty minutes before eating, and start again an hour after.
Put the broth on days one to three
Ozempic's peak days are the lowest for food and fluid. A cup of salted broth on each of those days covers the sodium the meals are no longer bringing.
A 2 litre (68 fl oz) day, with the protein carried in it
- On waking: 500 ml (17 fl oz) of water before anything else, then coffee or tea
- Mid-morning: a shake, 30 g scoop in 300 ml (10 fl oz) of milk, about 33 g of protein and a good share of the morning's fluid
- Through the afternoon: 750 ml (25 fl oz) of water or tea, sipped, plus a cup of salted broth on any day the food is under 1,000 calories (4,200 kJ)
- Evening: 250 ml (8.5 fl oz) of milk or a glass of water, with dinner finished an hour before, not a full bottle at 10pm
Around 2 litres (68 fl oz), a third of it carrying protein, with the salt built in on the low-food days. More in heat or after a session, and less pressure on the stomach because none of it lands on top of a meal.
