Is walking enough exercise on Saxenda? What it does and does not do

Walking is excellent and it is not enough on its own. It looks after your heart, your mood, your blood sugar and your daily energy use, and on Saxenda it does almost nothing to stop the muscle going with the fat, because it never asks a muscle to work near its limit. Keep the walking. Add two 30-minute strength sessions a week across six movements, with a load that makes the last two repetitions hard, and you have covered both jobs in an hour a week. If you have a heart condition, get your doctor's go-ahead before adding load.

Why steps do not protect muscle

Muscle is kept when it is asked to produce force it finds difficult. Walking asks the legs for a small effort, thousands of times, and the body adapts to that by getting better at walking, not by keeping tissue it no longer needs for anything heavier. In a large calorie deficit that distinction matters. Pooled across 20 randomised trials, 26.8% of the weight lost on liraglutide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. Nobody counted in the first two figures was given a training programme, and plenty of them will have walked.

There is a second problem particular to Saxenda. Liraglutide's appetite effect is milder and steadier than the weekly medications. Most people we coach on Saxenda can eat a normal small meal; the risk is subtler, because the appetite that remains is often for toast rather than for chicken. A long walk on a day when you have eaten 600 calories (2,500 kJ) and 30 g of protein is a further withdrawal from an account that is already overdrawn. Walking does not cause muscle loss, but the combination of a lot of it, very little food and no strength signal is exactly the profile of the people we coach whose sit-to-stand count is falling while their step count climbs.

On daily Saxenda the walks can fall wherever the day allows. Keep the two strength sessions outside the peak, eight to twelve hours after the injection, and eat protein before both.

Keeping the walks and adding the hour that counts

  1. Keep walking, and feed it

    Thirty to sixty minutes most days is a good habit and a good use of a GLP-1's energy. Take 20 to 30 g of protein within an hour either side of a long walk, particularly on the days after an injection when food is lowest.

  2. Add two 30-minute strength sessions

    Squat pattern, hinge, push, pull, carry, core brace. Two sets of each. Dumbbells, a band and a chair at home, or six machines at a gym. At least two days apart.

  3. Make the last two repetitions hard

    This is the difference between exercise and a strength signal. If you finish a set of 10 and could do 10 more, the load is too light to keep anything. Add weight until the last two are difficult and the form is still clean.

  4. Turn one walk into a loaded walk

    Carry a weight in each hand for the last 100 metres (110 yards) of a walk, or wear a loaded backpack on hills. It is not a substitute for the sessions, and it turns a walk into something the legs and grip have to answer.

  5. Keep the sessions outside the daily peak

    Saxenda peaks eight to twelve hours after each injection. Walk whenever suits; put the two strength sessions in the hours furthest from the peak, fed on both sides.

A week that covers both jobs

  • Monday, Wednesday, Friday, Sunday: a 30 to 45 minute walk, with 20 to 30 g of protein within an hour of finishing
  • Tuesday: strength session A, about 30 minutes: goblet squat, dumbbell row, push-up, suitcase carry, plank
  • Saturday: strength session B, about 30 minutes: hip hinge, shoulder press, band pulldown, split squat, dead bug
  • Every day: protein at 1.2 to 1.6 g per kg of goal weight (0.55 to 0.7 g per lb), in three or four servings of 30 to 40 g

Check with your doctor before adding load if

  • You have a heart condition, uncontrolled blood pressure, or you get chest pain, breathlessness or dizziness when you walk
  • You have had a fall, a fracture or surgery in the last year, a joint replacement, or a back or joint you have been told to protect
  • You take insulin or a sulfonylurea, where long walks on very little food can drop blood sugar and the plan needs agreeing with them

The three numbers that tell you what you are keeping

A rising step count feels like progress and tells you nothing about muscle. These three do.

  1. One strength test, same way every week

    Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.

  2. Average protein a day, whole week

    Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.

  3. Upper arm tape, same spot, relaxed

    Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.

Questions people ask

Is walking enough to keep your muscle on Saxenda?

No. Walking is good for your heart, mood and blood sugar, and it does not load muscle enough to keep it in a large calorie deficit. Two 30-minute strength sessions a week, with a load that makes the last two repetitions hard, is the part that protects muscle.

How many steps a day should I aim for?

Whatever you can sustain, and it matters less than people think. Thirty to sixty minutes of walking most days is plenty. The number that predicts whether you keep muscle is the strength test, not the step count.

Is walking enough on Saxenda?

No. Walking is good for you and does not load muscle enough to hold it in a deficit. Keep the walks and add two 30-minute strength sessions a week, timed away from the daily peak.
