Coming off Zepbound: how to keep the weight off

What happens when the medication stops

In SURMOUNT-4, people switched from tirzepatide to placebo regained a large share of what they had lost, roughly two thirds on average, over the following year. Appetite comes back, food noise comes back, and stomach emptying returns to normal within a few weeks of the last dose. None of that is failure.

What decides how much comes back is largely what your body is made of when you stop. Pooled across 20 randomised trials, 25.4% of the weight lost on tirzepatide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. Muscle sets how much energy you burn at rest and how much you can eat without regaining; lose a large share of it and you stop with a smaller engine and a returning appetite.

Where loss has been fast on Zepbound, the strength test often has the most catching up to do before the last dose. Start now: six weeks of two sessions a week usually recovers a dropped sit-to-stand count, and that is the engine you keep when the medication goes.

What to measure instead of trusting the scale

Keep these going for a year after the last dose. They tell good regain from bad.

  1. One strength test, same way every week

    Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.

  2. Average protein a day, whole week

    Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.

  3. Upper arm tape, same spot, relaxed

    Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.

The step-down plan, starting three months out

  1. Rebuild the muscle now, not after

    If your strength test has dropped since you started, the last three months on the medication are the time to reverse it. Two 30-minute sessions a week, protein at 1.6 g per kg of goal weight (0.7 g per lb).

  2. Set a weight range, not a weight

    Pick a 3 kg (7 lb) band and weigh weekly. Touch the top of it and the response is two weeks of protein and training, not panic.

  3. Keep the three numbers going for a year

    Strength test, protein average, arm tape, every Sunday. Regain with strength climbing is a different problem from regain with strength falling.

  4. Plan the first month off

    Hunger returns before habits adjust. Meals at fixed times, protein first then vegetables then the rest, plate size fixed, no snacking after dinner. Expect a 1 to 2 kg (2 to 4 lb) water bounce in week one; it is not fat.

  5. Practise eating at maintenance while still on it

    Zepbound users often go straight from 800-calorie days to full appetite. Spend the last month eating a full maintenance plate, protein first, so the habit exists before the hunger does.

A maintenance day at 130 g for a 75 kg (165 lb) goal

  • Breakfast: 2 eggs and 150 g (5 oz) Greek yoghurt: about 27 g
  • Lunch: 150 g (5 oz) chicken or 200 g (7 oz) tofu on a big salad with olive oil: about 42 g
  • Afternoon: 30 g (1 oz) nuts and 100 g (3.5 oz) cottage cheese: about 18 g
  • Dinner: 150 g (5 oz) fish or lean meat, 150 g (5 oz) vegetables, 100 g (3.5 oz) potato or rice: about 38 g

About 130 g and a normal amount of food. Maintenance is a fuller plate with the same protein rule, not a smaller deficit.

Will we regain the weight after stopping Zepbound?

Three months: long enough to rebuild a dropped strength test, fix the protein habit and practise maintenance eating while the medication is still helping.

The evidence, and how current it is

What comes back is not what came off. That is the single most useful thing to know before your last dose.
