Weight loss stalled after 3 months on Zepbound: what to check first
A stall at three months on Zepbound is common, and the first thing to check is not the dose but your three numbers: is the strength test holding, is protein at target, is the waist shrinking while the arm holds. If they are right, a flat scale for two to four weeks is often fat loss hidden by water and rebuilt muscle; if not, fix protein and training before anyone touches the medication.

The people we most often hear from about a stall on Zepbound are the ones whose first three months moved quickly. The body has a lot to adjust to. Three weeks of steady numbers before you decide anything.
The stall-breaker day: same calories, more protein
- Breakfast: 200 g (7 oz) Greek yoghurt with berries, not toast and butter: about 20 g
- Lunch: 150 g (5 oz) chicken on a large salad, not a sandwich: about 45 g
- Afternoon: a shake with 250 ml (8.5 fl oz) milk, not a muesli bar: about 30 g
- Dinner: 150 g (5 oz) fish or lean mince with vegetables, not pasta: about 35 g
About 130 g. Not fewer calories: the same calories with protein in place of the drift foods that crept back at month three.

Zepbound is usually started low and stepped up at intervals of several weeks, and there is more than one step your prescriber can settle you on. By three months most people are near their working dose, the early water loss is gone, and the body has adjusted to eating less. Weight loss in the trials was front-loaded: fast for the first few months, then slower across the year. A slower month three is the trial curve, not a failure.
The second reason is the one to look at: if you have started strength training or your protein has come up, you may be adding muscle while losing fat, and the scale hides it. Pooled across 20 randomised trials, 25.4% of the weight lost on tirzepatide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. Someone who lost 12 kg (26 lb) with 3 kg (7 lb) of it muscle and someone who lost 10 kg (22 lb) of fat look identical on the scale.
The three checks before you panic
Strength test trend, four weeks
If your sit-to-stand or goblet squat count is level or rising, you are not losing muscle. A stall with rising strength is a good stall.
Protein average, logged for real
A week of weighing food. In the stalls we look at, protein is under the floor about half the time, and snacks have crept back that carry more calories than the small meals did.
Tape: waist versus upper arm
Waist down 1 to 2 cm (0.5 to 1 in) in a month with the arm steady means fat is going and muscle staying. Both flat is a true plateau. Arm down and waist flat is the pattern to act on, and that is the one to fix.
Then, and only then, talk to your prescriber
Bring four weeks of numbers. Whether to change the dose is their call; your job is to arrive with data.
Give the body three weeks to catch up
A fast first three months is often followed by a stall while the body adjusts. Hold protein and training for three weeks before you decide anything.
How you will know it is working
A stall is only bad news if these three say so.
One strength test, same way every week
Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.
Average protein a day, whole week
Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.
Upper arm tape, same spot, relaxed
Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.

Questions people ask
Is a plateau at 3 months on Zepbound normal?
Yes. Loss in the trials slowed after the first few months. Two to four flat weeks with strength holding and the waist shrinking is progress, not a plateau.
Should we eat less to break a stall on Zepbound?
Usually not. Most stalls we see come with protein under the floor and snacks creeping in. Fix protein first, hold calories, and give it three weeks.
Why has our Zepbound weight loss stopped after 3 months?
Usually a mix of water, the body adjusting to fast loss, and drift foods returning. Log a week, check the strength test, and hold for three weeks.
What the evidence actually says
A stall is not the problem. A stall with a falling strength test is, and the first figure explains why a scan may not show it.
