Muscle cramps on Zepbound: causes and what to change
Cramps on Zepbound usually come from eating and drinking much less than you used to: less fluid, less sodium, less potassium and less magnesium arriving each day, sometimes with extra losses from vomiting or loose stools. Low protein and lost muscle make the problem worse, because there is less trained tissue doing the same work. Cramps that keep coming despite food and fluid, or that arrive with numbness, weakness or dark urine, belong with your prescriber rather than with a coach.

Zepbound users lose quickly, and fast loss on very low intake is where cramps and low strength turn up together. If both are present, treat it as a protein and training problem as well as a fluid one.
A day that covers fluid, salt and minerals
- Breakfast: Greek yoghurt 200 g (7 oz) with kiwifruit and a tablespoon of pumpkin seeds, about 22 g protein plus potassium and magnesium
- Lunch: baked potato with the skin on, 120 g (4 oz) tuna and salt, about 33 g and a solid potassium serve
- Afternoon: 250 ml (8.5 fl oz) milk, about 8 g, plus a glass of water
- Dinner: 120 g (4 oz) chicken with spinach and beans, about 40 g, seasoned properly
Around 103 g of protein, with the minerals coming from food rather than a tub. If cramps persist for more than a week or two on this pattern, that is a prescriber conversation, not a coaching one.

A muscle contracts and relaxes on a supply of fluid and minerals. On Zepbound you are eating perhaps half the food you were, so sodium, potassium and magnesium are all arriving in smaller amounts, and most people also drink less because food is where a surprising share of daily fluid comes from. Add a hot day, a training session or a bout of loose stools and the margin disappears. Tirzepatide is the medication where people most often describe a very short appetite window: a few bites, then done. Zepbound users tell us the food noise goes quiet almost entirely, which feels like freedom and removes every reminder to eat.
The second layer is the muscle itself. Pooled across 20 randomised trials, 25.4% of the weight lost on tirzepatide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. Muscle that is shrinking and untrained fatigues sooner during ordinary activity, and a fatigued muscle cramps more readily. That is why the people we coach who cramp the most are usually the ones lowest on protein, not the ones training the hardest.
What to change this week
Put the fluid back, deliberately
Around 2 litres (68 fl oz) a day, more in heat or after training. Set it out in front of you, because thirst is an unreliable prompt when you are barely eating. Water counts, tea counts, milk counts twice because it brings protein.
Stop avoiding salt
Low intake means low sodium, and low sodium is one of the commonest reasons for night cramps in the people we coach. Salt your food normally. If your prescriber has told you to limit sodium for blood pressure or kidney reasons, follow them, not us.
Eat the potassium and magnesium foods rather than guessing at pills
Potato with skin, banana, kiwifruit, yoghurt, milk, beans, spinach, almonds, pumpkin seeds. Ask your prescriber before starting any supplement, especially if you take blood pressure or heart medication.
Get protein to the floor and keep the two sessions
1.2 g per kg of goal weight (0.55 g per lb) as the minimum. For a 70 kg (154 lb) goal, 84 g a day. Two 30-minute strength sessions a week keep the muscle conditioned for the work you ask of it, which is the slower half of the fix.
Pair the fluid fix with the strength test
Zepbound loss is fast enough that cramps and weakness often appear in the same fortnight. Do the 30-second sit-to-stand this Sunday so you know which problem you are solving.
How you will know it is working
Cramps that come from lost muscle look different in the numbers from cramps that come from lost fluid.
One strength test, same way every week
Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.
Average protein a day, whole week
Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.
Upper arm tape, same spot, relaxed
Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.
See your prescriber about cramps that do not settle
- Cramps that keep coming after a week or two of proper fluid, salt and food
- Cramps with numbness, tingling, persistent weakness, swelling in one leg, or dark or cola-coloured urine, which need same-day attention
- Any cramping while you take diuretics, blood pressure medication, statins or laxatives, or if you have a kidney, heart or thyroid condition

Questions people ask
Does Zepbound cause muscle cramps?
Cramps are not a headline side effect of the medication itself. They are common in people whose intake has dropped a long way, because fluid, sodium, potassium and magnesium all fall with the food. If they persist once you have fixed that, see your prescriber.
Are cramps a sign I am losing muscle on Zepbound?
They can be part of the picture, particularly alongside a falling strength test and protein under the floor. On their own they point more often at fluid and minerals. Check both.
Do cramps mean I should lower my Zepbound dose?
Never change your dose on your own. Take the cramps, your food log and your fluid intake to your prescriber and let them decide. In the meantime, salt, water, potassium foods and protein at the floor.

Cramps are usually fluid and salt. If they persist, this is the list of markers worth raising with your prescriber.
