No appetite on Zepbound: what to eat

When Zepbound has switched your appetite off, eat protein first at three fixed times a day and let the rest of the plate be optional. Aim for 30 to 40 g of protein at each (a palm-sized portion of meat, fish or tofu, or 200 g / 7 oz of Greek yoghurt), whether or not you feel like it.

Your number, before anything else

Protein is what decides whether the weight you lose is fat or muscle. Work your number out once and we will keep it with you on every page here. No account, no email.

Why the appetite goes, and why your muscle cares

Tirzepatide acts on two gut hormone pathways, GLP-1 and GIP. It slows stomach emptying, brings fullness on early, and for many people switches off food noise almost completely. Tirzepatide is the medication where people most often describe a very short appetite window: a few bites, then done. Zepbound users tell us the food noise goes quiet almost entirely, which feels like freedom and removes every reminder to eat.

The trouble is that hunger used to remind you to eat protein. Without it, most people we coach drift to 50 or 60 g a day within weeks, because toast and tea are easy and a piece of chicken is not. Pooled across 20 randomised trials, 25.4% of the weight lost on tirzepatide was lean mass, against 26.2% when the same weight came off by diet alone, and 17.5% when resistance training was added. Not eating is how that number happens to you.

Zepbound's dual action can turn food noise off almost completely, which feels like freedom and quietly removes every reminder to eat. Alarms on your phone are not overkill in the first few months.

What to eat this week when nothing appeals

  1. Eat by the clock, not by hunger

Three anchor times: within an hour of waking, midday, and early evening. Set alarms. Hunger will not tell you; the clock will.

  2. Protein first, and small

A 120 g (4 oz) portion of cooked chicken, fish or lean mince is about 30 to 35 g of protein. If that is too much, halve it and add 200 g (7 oz) of yoghurt or a glass of milk two hours later.

  3. Make it cold, soft or drinkable

Cold and soft foods go down more easily on Zepbound: cottage cheese, yoghurt, tinned tuna, boiled eggs, a shake made with 250 ml (8.5 fl oz) of milk, which is 25 to 35 g of protein for about 250 calories (1,050 kJ).

  4. Drink 2 to 2.5 litres (68 to 85 fl oz)

Low appetite usually means low fluid too, and dehydration makes the foggy feeling worse. Milk, soup and shakes count.

  5. Set the alarms before the food noise goes

Zepbound can remove the thought of food entirely. Three phone alarms, labelled with what to eat, replace the hunger signal that used to do that job. It sounds excessive until the first week you forget lunch twice.

A day that hits 100 g of protein on a small appetite

  • Morning: 200 g (7 oz) Greek yoghurt with a scoop of protein powder stirred in: about 40 g
  • Midday: 95 g (3.5 oz) tin of tuna with a boiled egg: about 28 g
  • Evening: 100 g (3.5 oz) cooked chicken or 150 g (5 oz) firm tofu: about 30 g
  • If a meal is missed: 250 ml (8.5 fl oz) milk with a scoop of protein powder: about 30 g

Roughly 100 g for 700 to 800 calories (2,900 to 3,350 kJ). Adjust to your own target from the calculator.

Forty foods, ranked by protein per bite

The hard part is not knowing you need protein on Zepbound, it is hitting the number on a day when food is the last thing you want. One page, grams and ounces, what a real serve looks like, and the first-bite rule for when appetite is gone. Print it and put it on the fridge.

Then five short emails over five days on keeping muscle while the weight comes off, and a short note every fortnight after that if you want it. One click to stop, any time.

When to talk to your prescriber

  • You cannot keep fluids down, or you go more than a day eating almost nothing
  • You are losing more than 1.5 kg (3 lb) a week for several weeks running
  • You feel faint or dizzy, or your heart races when you stand
  • Anything about dose or timing. Zepbound decisions are for your prescriber

Dose and timing on Zepbound are your prescriber's decisions; we coach the food and the strength.

The three numbers that tell you what you are keeping

Appetite is not a reliable gauge on Zepbound. These three are.

  1. One strength test, same way every week

Goblet squat reps at a fixed weight, push-ups, or the 30-second sit-to-stand for beginners. Fat loss does not make you weaker, so a number that drops three or four weeks running is a real signal, and it is fixable. It measures what you can do, not how much muscle you have.

  2. Average protein a day, whole week

Not your best day. 1.2 to 1.6 g per kg of goal body weight (0.55 to 0.7 g per lb), floor 1.2, protein first at every meal.

  3. Upper arm tape, same spot, relaxed

Centimetres or inches. The waist shrinks whatever you do; if the arm shrinks as fast, that is muscle. Weight is number four, same day and time. The scale tells you what you lost. These tell you what you kept.

Questions people ask

Should I force myself to eat on Zepbound if I have no appetite?

Eat protein by the clock, small and often, and let the rest be optional. Forcing a full plate usually ends in nausea; three small protein anchors a day does not.

What is the best breakfast on Zepbound when we cannot face food?

Something cold and drinkable. 200 g (7 oz) of Greek yoghurt, or milk with protein powder, is 25 to 40 g of protein without needing an appetite.

We forget to eat on Zepbound. Is that a problem?

It is the most common way protein collapses on tirzepatide. Forgetting is not hunger returning later; the meal is simply gone. Alarms and pre-made protein in the fridge are the fix.

The research behind this page

When you can only manage a few bites, the size of each one decides whether it counts. These are the two numbers that set it.

Spread across meals at roughly 0.3 to 0.4 g per kg per meal, rather than taken mostly at dinner. Arslan S, Clinical Nutrition ESPEN 2026

Each of those meals wants roughly 2.5 to 3 g of leucine, the amino acid that actually switches muscle building on. That is about 30 g of whey, 120 g of chicken breast, or four eggs. Arslan S, Clinical Nutrition ESPEN 2026
